J.L. Pottinger DDS Membership Plan
Terms and Conditions
This Dental Membership Plan is NOT dental insurance; it is a loyalty membership discount plan offered by Jennifer L. Pottinger DDS,PA. If periodontal disease is present, additional treatment not covered under the plan will be necessary prior to your cleaning. The Plan and any discounts included do not cover procedures performed outside of our practice.
- As long as your Membership Fee payments are current, you will not be charged for the included services. You are obligated to pay Jennifer L. Pottinger DDS directly for an Non-included Services that Jennifer L. Pottinger DDS provides for you.
- You must be current on all Membership Fees for the Dental Membership Plan in order to receive any Included Services or discounts for any Non-Included Services.
- You may not combine our Dental Membership Plan with any other discounts, discount plans, dental insurance (including Medicare, Medicaid, other forms of government insurance or assistance, and/or any private insurance plan or policy), or in-office promotion, or in conjunction with Care Credit.
- It is possible that, during the course of treatment by your dentist, she may discover a dental or health condition, such as periodontal disease, that may require further treatment-the costs associated with any further treatment are your responsibility.
- Discounts provided under the Dental Membership Plan apply only to services provided by Jennifer L. Pottinger DDS and not products that Jennifer L. Pottinger DDS may sell or services rendered by a dentist other than those at Jennifer L. Pottinger DDS.
- Any late fees or missed appointment fees that the Dentist may charge will still apply and will be your responsibility.
- Discounts from the Dental Membership Plan are not retroactive for previous procedures prior to the date of joining the membership.
- The Membership Plan is for 365 days from the date of joining. Annual fee is due in full on the start date.
- In all 50 states, to offer a discount to non-insured patients, the office must collect up front for services rendered. This means the office must collect fees up front at time of service and NO AFTER APPOINTMENT BILLING.
